Overview

Prostate cancer is one of the most common cancers in men, starting in the prostate, a small gland that sits just below the bladder. This gland plays an important role in reproduction by producing fluid that nourishes and transports sperm.

What makes prostate cancer complex is how differently it behaves from person to person. Some tumours grow very slowly and can be monitored closely. Others are more aggressive and need prompt treatment right away. 

While most cases progress slowly, early detection significantly improves outcomes.

That’s why the first step is a careful assessment of risk and stage. At SSO Hospitals, we use a structured approach to diagnose your condition clearly and then recommend the most suitable treatment path for your specific case.

Symptoms and signs of prostate cancer

Early prostate cancer often causes no symptoms. Many cases are detected during routine screening, such as a PSA blood test and a prostate exam. When symptoms do appear, they can also be linked to other conditions, such as an enlarged prostate or urinary infections. 

If you notice prostate-related symptoms that persist, worsen, or feel unusual for you, it is best to get them evaluated. 

  • Difficulty starting urination or a weak urine stream
  • Frequent urination, especially at night
  • Sudden urge to urinate
  • Pain or burning while urinating
  • Blood in urine or semen
  • Pain in the lower back, hips, or pelvis
  • Bone pain (a sign cancer has spread)

Types
Diagnosis

Staging helps understand how far the cancer has grown or spread, and how it may behave over time. We consider your cancer stage along with your PSA, biopsy results, and scan findings to create a personalised treatment plan:

Stage I:
Cancer is small and confined to the prostate, growing very slowly.

Stage II:
Cancer is still within the prostate, but larger or higher grade.

Stage III:
Cancer has spread beyond the prostate to nearby tissues like the seminal vesicles.

Stage IV:
Cancer has spread to lymph nodes, bones, or other distant organs. Your treatment focuses on controlling growth and managing symptoms.

LMost prostate cancers are adenocarcinomas. These develop in the gland cells that line the prostate and produce prostate fluid. Nearly all men diagnosed with prostate cancer have this type.

Adenocarcinomas have two main subtypes:

Acinar adenocarcinoma is the most common form and arises from glandular cells.

Ductal adenocarcinoma arises from the cells lining the prostate ducts and tends to grow more rapidly.

There are also two rare types of prostate cancer: small cell carcinoma, which grows aggressively and quickly, while squamous cell carcinoma develops from the flat cells covering the prostate surface.

We start by understanding your symptoms, then recommend screening tests to detect any signs of abnormal cell growth. If anything suspicious arises, we proceed to other diagnostic procedures.

Here are the tests you can expect in your prostate cancer examination:

PSA blood test: This measures a protein produced by your prostate.

Digital rectal exam (DRE): Your doctor physically examines the prostate through the rectum wall, checking for lumps, hard areas, or unusual texture that might indicate cancer.

MRI scan: If initial tests raise concerns, an MRI provides detailed images of the prostate to identify suspicious areas and guide further testing.

Biopsy: The definitive way to confirm cancer. A thin needle removes small tissue samples from the prostate to check for cancer cells.

After the biopsy, we grade the cancer's aggressiveness on a scale of 6 to 10, with higher numbers indicating faster growth.

Risks and Complications

There is no exact cause of prostate cancer in most cases. But we can tell you that certain factors increase risk, which helps us decide who may need closer evaluation.

  • Age: Risk rises steadily after 50, and increases further with age.
  • Family history: A close relative with prostate cancer raises your risk.
  • Inherited gene changes: Mutations in genes like BRCA2 and BRCA1, as well as other inherited mutations, can increase risk.
  • Ethnicity and ancestry: Risk is higher in men with Black ancestry.
  • Lifestyle factors: Weight, diet, and activity may influence risk, though the links are less consistent than age and genetics.
  • Obesity: Excess weight is linked to aggressive forms

When is surgery recommended?

Surgery is often recommended for localised prostate cancer that has not spread beyond the gland. The procedure, called radical prostatectomy, removes the entire prostate and some surrounding tissue. It is most effective when the cancer is confined to the prostate and removal is likely to be curative. 

We consider several other factors, such as age, overall health, cancer stage, Gleason score, and personal preferences, before recommending surgery.

In some cases, surgery is part of a combined plan and may be followed by radiation or other therapy based on final biopsy findings. It’s not suitable for advanced cancer that has spread to lymph nodes or bones. In such cases, we recommend radiation therapy, hormone therapy, or chemotherapy instead.
