Overview

Cervical cancer develops in the cells of the cervix, the lower part of the uterus that connects to the vagina. It typically develops slowly over time, often starting from precancerous changes.

The challenging part is that early cervical cancer rarely causes noticeable symptoms. Most women don’t realise anything is wrong until the cancer has started to spread. This is exactly why regular screening tests and HPV vaccines are so important for prevention.

When cervical cancer is caught early through screening, treatment is often very successful. Early detection gives you more treatment options and significantly better outcomes. Regular checkups can catch changes before they turn into cancer, making prevention possible.

Symptoms

As the cancer progresses, you may notice specific changes in your body. It’s essential to pay attention to these signs so you can seek medical care promptly.

  • Abnormal vaginal bleeding between periods, after intercourse, or after menopause
  • Unusual vaginal discharge that may be watery, bloody, or have a foul odour

Stages
Types
Diagnosis

Staging shows how far cancer has progressed and helps predict your prognosis. Your treatment options expand or narrow depending on what stage you're at when diagnosed.

Stage I:
Cancer stays within the cervical surface and hasn't invaded deeper tissue

Stage II
The growth extends past the cervix into surrounding tissue but stops before the pelvic wall.

Stage III:
Cancer reaches the pelvic wall, lower vagina, or blocks urine flow to the kidneys.

Stage IV
The tumour has spread to the bladder, rectum, or distant organs like the lungs or liver.

Not all cervical cancers are the same. Where the cancer originates in the cervix determines its type and influences treatment decisions. Some types grow more aggressively than others, which is why identifying the specific cell type matters for your care plan.

Squamous cell carcinoma: This tumour begins in flat, thin cells on the outer cervix and accounts for most cases.

Adenocarcinoma: Cancer originates in mucus-producing glandular cells lining the cervical canal.

Adenosquamous carcinoma: It has characteristics of both squamous and glandular cells, making it less common.

Small cell carcinoma: A neuroendocrine tumour that releases hormones, grows rapidly, and often spreads early to other organs.

We start with a pelvic exam and ask about any symptoms you've been experiencing. If something seems off, we'll run more tests to get a clear picture. The goal is to confirm whether cancer is present and understand exactly what we're dealing with.

Pap smear: A screening test where cells are gently scraped from your cervix and examined under a microscope for precancerous or cancerous changes.

HPV test:Often done alongside a Pap smear, this test identifies specific high-risk HPV strains that can lead to cervical cancer.

Colposcopy:If your Pap results show abnormalities, a colposcope magnifies your cervix so doctors can spot unusual areas and take targeted biopsies.

Biopsy:A small piece of cervical tissue is removed and sent to a lab where pathologists examine it to confirm whether cancer cells are present.

Imaging tests:CT scans show detailed cross-sections of your pelvis, MRIs reveal soft tissue involvement, and PET scans detect cancer spread throughout your body.

When surgery becomes necessary

Surgery is often the best option for early-stage cervical cancer or precancerous changes that need to be removed. We’ll recommend a surgical approach based on how far the cancer has progressed and what matters most to you. 

If preserving your fertility is important, we’ll explore options that protect your ability to have children while effectively treating the cancer. 

For very early cases, surgery alone might be all you need to get back to health. In other situations, we may combine surgery with radiation or chemotherapy for the best possible outcome.

      Can cervical cancer be prevented?

      Yes, through HPV vaccination and regular screening with Pap smears and HPV tests throughout your life.

      How often should I get screened?

      If between 21 and 29, get a Pap smear every three years. HPV testing isn’t routine at this age. From 30 to 65 pick one option: a Pap smear every three years, an HPV test every five years, or both tests together every five years. After 65, you can stop screening with normal results and no precancerous changes (CIN 2+) in the past 20 years. 

      Recommended Screening Schedule (Standard Guidelines)

      Age 21–29 years

      • Pap smear alone every 3 years
      • HPV testing not routinely recommended

      Age 30–65 years

      Any ONE of the following:

      • Pap smear alone every 3 years, OR
      • HPV test alone every 5 years, OR
      • Co-testing (Pap + HPV) every 5 years 

      Above 65 years

      • Screening can be stopped if:
        • Adequate prior negative screening, AND
        • No history of CIN 2+ in last 20 years
      Is cervical cancer curable?

      Cervical cancer is often curable when detected early and treated on time. However, treatment outcomes can still vary depending on your personal health and stage, with chances of recurring.

      Does HPV always cause cervical cancer?

      No, most HPV infections clear naturally within two years. Only persistent infections with high-risk types may lead to cancer over time.
